Information for New Clients:
Prior to the start of our first meeting you will need to complete the appropriate "New Client Paperwork Packet". Please visit the Forms section if you wish to download the packet to fill out at home. You may also opt to arrive 20-30 minutes prior to the start of your first appointment, and the forms will be made available for you in the waiting room. If you do arrive early to complete the paperwork a clipboard with the forms will be left for you in the waiting room; if more than one clipboard is out there please look for the one with a note on the front with my name on it.
Please note that if therapeutic services are for a minor (0-17yrs), then the first appointment is for Parents Only. This invaluable time will give us an opportunity to discuss goals, concerns, and other important developmental information.
What to expect on your first visit:
Complete paperwork and discuss Informed Consent (what to expect from therapeutic process)
Discuss therapeutic goals and areas of concern
Discuss pertinent relationship, mental health, and physical health history
Get to know each other-the client-therapist relationship is one of the most important healing factors in therapy!
Don’t forget to bring your paperwork, photo ID, method of payment, insurance card (if applicable), and divorce decree (if applicable, see child packet information below). You are welcome to bring any supplemental materials you might feel to be helpful (e.g IEP plans, 504 plans, psychological evaluations, etc.)
If you are interested in Teletherapy services we will discuss the appropriateness of such services in relation to your goals. If deemed appropriate you will receive a brief training on how to use the video platform and best practices for Teletherapy.
