New Client Information

Please complete the appropriate information packet prior to your first appointment. To open and print the forms simply click on the bulleted words. Should you choose to fill out the packet in office, please make sure you arrive 30 minutes before your first scheduled appointment starts.

Information for New Clients:

 

Prior to our first session you will need to complete the appropriate New Client paperwork. All forms can be completed electronically through your Therapy Appointment client portal account (see Therapy Appointment Tab for more info). Once you are set up you can access the portal anytime to view upcoming appointments, request appointments, message me, complete forms and share documents, and access your billing records.

Please note that if therapeutic services are for a minor (0-17yrs), then the first appointment is for Parents Only. This invaluable time will give us an opportunity to discuss goals, concerns, and other important developmental information. 

***Special Notice: Prior to children participating in therapy, the most current divorce/custody decree is required to be on file. If applicable please upload a copy of your decree onto the Therapy Appointment portal.

What to expect on your first visit:

  • Discuss the Informed Consent paperwork signed (what to expect from therapeutic process)

  • Discuss therapeutic goals and areas of concern

  • Identify strengths

  • Discuss pertinent relationship, mental health, and physical health history

  • Get to know each other-the client-therapist relationship is one of the most important healing factors in therapy!

  • If you are interested in Teletherapy services we will discuss the appropriateness of such services in relation to your goals. If deemed appropriate you will receive a brief training on how to use the video platform and best practices for Teletherapy.
