Private Pay

My Private Pay rate for a standard 50 minute session is $160. I am happy to provide service invoices, upon request, for you to submit to your insurance company for out-of-network benefits.

Good Faith Estimate (Applicable to Private Pay Clients Only)

You have the right to receive a “Good Faith Estimate.” Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for non-emergency medical items and services. This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. • The Good Faith Estimate does not obligate or require the client to obtain any of the listed services from the provider. Insurance

  • In-Network with Blue Cross/Blue Shield PPO and United Health Care PPO. Co-Pays and Co-Insurances Apply

  • Sessions can be 45 or 55 minutes long depending on client need

  • When you call to schedule please be prepared to provide the information listed on your insurance id card

  • Please make sure to upload a copy of your insurance card and photo ID onto Therapy Appointment

Payment

  • Cash, Checks, and Credit/Debit cards are accepted. Payment for service is due in full at the time service is rendered.

Cancellation and No Show Policy

  • 24 hour cancellation notice is required if you need to cancel or reschedule your appointment. In the event that you do not provide notice within 24 hrs or do not show up to your scheduled appointment, you will be required to pay the full cost of the session.

  • Insurance cannot be billed for cancellations/no shows-clients will be responsible for the full session fee
